Speaker Honorarium Process
Thank you for being a speaker for the FEED programming!
To receive your payment, please follow this process:
- Download a copy of the IRS W9 form. https://www.irs.gov/pub/irs-pdf/fw9.pdf
- Fill out page 1 of the W9 form. If you need help:

- Print page 1 of the W9 form.
- Mail to:

- Once the W9 is in the mail, please let us know. Email foodsystems@extension.wisc.edu to notify that the W9 is in the mail.
- We will mail you a check. Please note, that it sometimes takes up to 3 months for the check to arrive. We apologize for the delay in advance!
- Once you get the check, please let us know. Email foodsystems@extension.wisc.edu to notify us that you got the check.
